PURPOSE:
Awarded by the BOC on April 9, 2019 for an amount not to exceed$5,996,003.45. The BOC approved Change Order No. 1 on February 11, 2020 amending the original funding allocation to add HOST funds and to reduce the amount of SPLOST funds originally allocated. This contract consists of the construction of 5-foot wide sidewalks with 2-foot grass strips, along with curbs and gutters to fill in the voids between the existing segments of the sidewalks to provide connectivity along Flat Shoals Road form the intersection of Second Avenue to just past Whites Mill Road, Henderson Road from the intersection of Henderson Mill Road to the intersection of Lavista Road, and at Salem Road form the intersection of Panola Road to Fannin Drive. This request is to provide funds for the construction of a retaining wall needed to address design of a steep slope identified after the completion of grading work to eliminate potential erosion problems.

Recommend approval of Change Order No. 2 with:
Precision 2000, Inc., 2215 Lawson Way, Atlanta, GA 30341
Amount Not To Exceed: $77,840.00
and authorize the Chief Executive Officer to execute the change order and all associated documents.
