PURPOSE:
Awarded by BOC on July 14, 2020, for an amount not to exceed $186,900.00. On August 10, 2021, the BOC approved the 1st renewal for an amount not to exceed $383,700.00. This contract is for therapeutic services. This request is to exercise the 2nd and final renewal option.
The contract expired July 31, 2022 and the renewal was delayed until receipt of grant funds was confirmed. This substitute is submitted to:
1. Ratify the thirty-day emergency extension to August 30, 2022.
2. Request approval of the renewal as stated below.
