PURPOSE:

Awarded by the BOC on April 9, 2019 for an amount not to exceed $5,996,003.45. On February 11, 2020, the BOC approved Change Order No. 1 amending the original funding allocation to add HOST funds and to reduce the amount of SPLOST funds originally allocated. On August 11, 2020, the BOC approved Change Order No. 2 to increase contract funds in an amount not to exceed $77,840.00 for the construction of a retaining wall needed to address design of a steep slope identified after the completion of grading work to eliminate potential erosion problems. On March 9, 2021, the BOC approved Change Order No. 3 to increase contract funds in an amount not to exceed $85,000.00 to allow for the excavation of numerous additional test holes to better determine the location of utilities that are in conflict with proposed storm drain systems along Henderson Road. On December 14, 2021, the BOC approved Change Order No. 4 to increase the contract term for an additional six (6) months to allow more time to address utility conflicts with proposed storm drain systems along Henderson Road. On June 14, 2022, the BOC approved Change Order No. 5 to increase contract funds in an amount not to exceed $310,462.95 to address costs related to the modified design for the water line and to address claims by the contractor for extensive unanticipated hand digging to locate the water line along Salem Road as well as increase the contract term for an additional six (6) months to allow more time to address utility conflicts with proposed storm drain systems along Salem Road. On December 13, 2022, the BOC approved Change Order No. 6 to increase the contract term for an additional six (6) months to allow the contractor time to complete the existing work.

This request is to increase the contract term through December 31, 2023 and funds to allow completion of a revised plan for the Henderson Road sidewalk in order to avoid utility conflicts.

 

RECOMMENDATION:

Recommended Action

Recommend approval of Change Order No. 7 with:

 

Precision 2000, Inc., 2215 Lawson Way, Atlanta, GA 30341

 

and authorize the Chief Executive Officer to execute the change order and all associated documents.
