PURPOSE:
Awarded by the BOC on March 8, 2022, for an amount not to exceed $2,400,000.00. This contract consists of furnishing all labor, materials, and equipment necessary to provide shotcrete and restorative services to construct concrete walls, pipes etc., on an as needed basis. This request seeks to increase the contract term and funds through February 28, 2027. This request will allow the contractor to continue providing the labor, materials, and equipment needed to perform shotcrete and restorative services for the construction of concrete walls, pipes, and related work on an as-needed basis.

Recommended Action
Recommend approval of Change Order No. 1 with:
Proshot Concrete, Inc, 4158 Musgrove Drive, Florence, AL 35630
Amount Not To Exceed: $1,500,000.00
and authorize the Chief Executive Officer to execute the change orders and all associated documents
