PURPOSE:
Awarded by the BOC on June 26, 2018 to two vendors for a total amount not to exceed $379,664.00 with an amount not to exceed $189,832.00 for Rem-Kiks. On June 25, 2019, the BOC approved Change Order No. 1 to the Rem-kiks contract for an amount not to exceed $110,224.85 for additional funding and ratification of payment for previously rendered services. On June 25, 2019, the BOC approved the 1st renewal option for an amount not to exceed $324,387.60. The contract for the second vendor was not renewed and funding for that vendor is not referenced herein.
RECOMMENDATION:
Recommended Action
Recommend approval to exercise the 2nd renewal option through June 30, 2021 with:
Rem-Kiks Health Care Services, Inc., 3448 Mountain Drive, Decatur, GA 30032
Amount Not To Exceed: $646,944.45
and authorize the Chief Executive Officer to execute the change order and all associated documents.
