PURPOSE:
Awarded on July 21, 2023, for an amount not to exceed $39,300.00. This contract consists of the purchase of a cloud-based software for the creation of various departmental forms (form builder) that includes training and support for Fire Rescue employees. This request seeks to ratify a forty-five (45) day contract term increase, effective August 1, 2024, and funds to establish software subscription service for four (4) years at a cost of $41,250.00 annually through July 25, 2028.

Recommend approval of Change Order No. 1 with:
GovOS Inc., 8310 N. Capital Texas Highway Bldg. 2, Suite 240, Austin, TX 78731
Amount Not To Exceed: $165,000.00
and authorize the Chief Executive Officer to execute the change order and all associated documents.
