Healthcare Access Expansion and Community-Based Health Services (Purchasing Policy Exemption). Consists of the provision of healthcare expansion and community-based health services to expand access to quality healthcare for uninsured and underinsured residents, including in-home primary care, dental and behavioral health services, community outreach, wellness services, immunization expansion, remote patient monitoring, and other initiatives to strengthen the DeKalb County healthcare ecosystem. A contract will be developed and executed in a form acceptable to the County Attorney or his designee. Awarded to: Mosaic Health Center, Inc. Amount Not to Exceed: $1,500,000.00.

PURPOSE:
Mosaic Health Center, Inc. has been engaged to expand access to healthcare services for uninsured and underinsured residents and strengthen the healthcare delivery system throughout DeKalb County. The scope of services primarily includes, but is not limited to, expanding in-clinic and in-home primary care services through the Mosaic Mobile program; increasing staffing and clinical capacity; providing dental, behavioral health, wellness, immunization, and remote patient monitoring services; conducting community health outreach and engagement activities; and supporting healthcare coordination and partnerships within the County.
NEED/IMPACT:
