PURPOSE:
Consists of providing professional service for the provision of in-home services (homemaker, personal care and in-home respite) and case management services. These services will be provided throughout the county to older adults, persons with disabilities and their caregivers who are residents of the county. Provision of these services will maintain and strengthen the capacity of functionally impaired individuals who would otherwise be unable to care adequately for themselves. Five (5) proposals received.
