PURPOSE:
Awarded by the BOC on December 9, 2014 for an amount not to exceed $798,905.00. On March 8, 2016, the BOC approved the Change Order No. 1 for an amount not to exceed $798,905.00. On November 15, 2016, the BOC approved the Change Order No. 2 for an amount not to exceed $798,905.00. This request is for additional funding and time needed on the contract to prevent a lapse in services as the new solicitation is being processed.

Recommend approval of Change Order No. 3 to increase the contract amount and extend the contract expiration date to December 31, 2018 with:
