PURPOSE:
Awarded on October 17, 2014 for an amount not to exceed $1,725.000.00. Amended May 11, 2016 to extend the contract term through June 30, 2018. This request is to increase funding and to extend the contract term for twelve (12) months to align the contract term with that of the County’s healthcare plan year and to acknowledge the company’s legal name change from Viverae, Inc. to SimplyWell, Inc.
RECOMMENDATION:
Recommended Action
Recommend approval of Change Order No. 1 to increase the contract amount and extend the contract term to June 30, 2019 with:
SimplyWell Inc., 10670 N. Central Expressway, Suite 700, Dallas, TX 75231
Amount Not To Exceed: $600,000.00
and authorize the Chief Executive Officer to execute the change order and all associated documents.
