PURPOSE:
Awarded by the BOC on January 14, 2015, for an amount not to exceed $300,806.00. This contract consists of providing lifeguard, aquatic instruction and pool maintenance services to seniors at the Lou Walker Senior Center. This request is to extend the contract term to April 30, 2020 and to increase the contract amount not to exceed by $25,000.00 to ensure services continue during the solicitation process. The solicitation is in final approval stage and should advertise in January 2020.

Recommended Action
Recommend approval of Change Order No. 1 with:
Exercise At Any Age, LLC, 3689 Cameron Hills Place, Ellenwood, GA 30294
Amount Not To Exceed: $25,000.00
and authorize the CEO to execute the change order and all associated documents.
