PURPOSE:
Awarded by the BOC on December 12, 2017 for an amount not to exceed $239,450.00. This software aids the County in providing web-based on-line claims intake from the field and claim data support. The Software-As-A-Service is proprietary, and the supplier is the only source that can provide the necessary service and support. This request is to extend the contract term to allow time to assess the evolving needs of the County.
RECOMMENDATION:
Recommended Action
Recommend approval of Change Order No. 1 to extend the contract term to December 31, 2021 with:
Origami Risk, LLC, 4360 Chamblee Dunwoody Road, Suite 111, Atlanta, GA 30341
Amount Not To Exceed: $99,715.00
and authorize the Chief Executive Officer to execute the change order and all associated documents.
