PURPOSE:
Awarded by the BOC on December 17, 2019 for an amount not to exceed $2,654,950.00. This contract consists of providing tiered assessments of the sanitary sewer system's manholes and small diameter pipes utilizing CCTV and acoustic inspections and smoke and dye testing; cleaning; as well as locating, adjusting, and replacement of manholes and covers. This request, to increase the scope of work through assistance with flow monitoring services; pre-CCTV inspection; and allowance replenishment, will allow the contractor to assist DWM in unforeseen critical services as well continue in providing existing sanitary sewer system tiered assessments.

RECOMMENDATION:
Recommended Action
Recommend approval of Change Order No. 1 with:

EnviroWaste Services Group, Inc. 18001 Old Cutler Road, Suite 554, Palmetto Bay, Florida 33157

and authorize the Chief Executive Officer to execute the change order and all associated documents.
