PURPOSE:
On December 12, 2017, the BOC approved the purchase of this software under a 3-year multi-year contract, for an amount not to exceed $239,450.00. On November 10, 2020, Change Order No. 1 for an amount not to exceed $99,715.00 was approved to extend the contract term to December 31, 2021. This Software-As-A-Service is proprietary, and the supplier is the only source that can provide the necessary service and support. This request is to establish a new five (5) year service agreement for the continuous servicing of the RMIS.

Origami Risk, LLC, 4360 Chamblee Dunwoody Road, Suite 111, Atlanta, GA 30341
Amount Not To Exceed: $476,700.00
and authorize the Chief Executive Officer to execute the contract and all associated documents.
