PURPOSE:
Awarded by the BOC on September 28, 2021 as a sole source in an amount not to exceed $412, 260.00. This request is for additional funding and to extend the expiration date to April 30, 2023. This will allow for the completion of the ADMS implementation project which has been delayed due to supply chain and staffing disruptions.

and authorize the Chief Executive Officer to execute the change order and all associated documents
