This contract consists of providing proprietary software for the Risk Management Information System (RMIS) used by the County in providing web-based reporting, tracking, and adjustment of all Property & Casualty and Workers Compensation Claims. PURPOSE:
Awarded by the BOC on December 14, 2021 for an amount not to exceed $476,700.00. On July 25, 2023, the BOC approved Change Order No. 1 for an amount not to exceed $89,802.50 for the addition of the Safety Management System (Environment, Health and Safety (EHS)). This Software-As-A-Service is proprietary and utilized by Risk Management for web-based reporting, tracking, and adjustment of all Property & Casualty and Workers Compensations Claims. This request is for the funding of the EHS module for the remainder of the contract term.

Recommend approval of Change Order No. 2 with:
Origami Risk, LLC, 4360 Chamblee Dunwoody Road; Suite 111, Atlanta, GA 30341
Amount Not To Exceed: $62,250.00
and authorize the Chief Executive Officer to execute the change order and all associated documents.
