PURPOSE:
Awarded by the BOC on July 11, 2017 for an amount not to exceed $747,000.00. On August 14, 2018, the BOC approved Change Order No. 1 for an amount not to exceed $249,000.00. This request is to add an additional funding and extend the contract term to December 31, 2021.

and authorize the Chief Executive Officer to execute the change order and all associated documents.
